Terms & Conditions

Before your first session you will be asked to sign this patient declaration:

 

1. I understand that Charlotte Taylor (C Taylor Personal Training) operates on a pre-pay basis and all sessions must be paid for in advance. Bookings will not be confirmed until payment has been received by Charlotte Taylor.

2. Should any change or cancellation be needed before 24 hours of my appointment; I understand that I will need to contact Charlotte Taylor before then, to either cancel coaching or change the services. Cancellations made with less than 24 hours notice will incur full charge.

 

3. Changes or cancellations of individual sessions within a month will need to be made within 24 hours of the session starting, they can be rescheduled to any other day within that month where a session is available. Any cancellations or reschedule attempts within 24 hours before the session starts, cannot be made and the full charge on the session will remain.

 

4. I understand all sessions will last either 30, 45 minutes or 1 hour unless otherwise stated and will start and finish on time. I also understand that Charlotte Taylor may not be able to provide a full session if I am late to a session. However, if Charlotte Taylor is late for a session, I will still receive the full session time or have the outstanding time added to another session.

 

5. I understand the results of any training programme or coaching course cannot be guaranteed and my progress depends on my effort and cooperation in and outside of the sessions. In particular I acknowledge that individual results may vary.

 

6. I certify that my answers to the questions outlined on any Pre-Activity Questionnaire form / consultation document or equivalent information are true and complete to the best of my knowledge. I understand and agree that it is my responsibility to inform Charlotte Taylor of any conditions or changes in my health, now and ongoing, which might affect my ability to exercise safely and with minimal risk of injury, or the adherence of any protocols pertaining to nutrition or supplementation.

 

7. I understand that Charlotte Taylor (C Taylor Personal Training) operates as a self-employed, independent, freelance personal trainer and sports therapist. I acknowledge that in engaging into this service I am entering into a contract with her alone and not with the establishment (Freedom Leisure) in which we train.
